SELF SELECT READING FORM

• Each month you are expected to read one book of your own choice.
• You must have my approval of the book before you begin to read it. 
• Approval is granted based on reading level and the student’s reasons for selecting the book.
• The QCS librarian would love to help you discover new and interesting literature.

Grading:

At the beginning of each month 5 points will be awarded after successfully submitting the submission form before midnight on the assigned day.

At the end of the month,  a maximum of 10 points will be awarded based on an oral book report.

The end of one month and the beginning of the next month fall on the same day,  so the grade for SSR is out of 15 possible points.
